A systematic review on implementation of person-centered care interventions for older people in out-of-hospital settings

of this study was to explore the content and essential components implemented person-centered care in out-of-hospital context for older people (65+). A systematic review conducted, searching published research electronic databases: PubMed, CINAHL, Scopus, PsycInfo, Web Science Embase between 2017 2019. Original studies with both qualitative quantitative methods were included assessed according quality assessment tools EPHPP CASP. The limited English, Swedish, Danish, Norwegian Spanish. In total, 63 original articles from 1772 hits. results final synthesis revealed following four interrelated themes, which are crucial implementing care: (1) Knowing confirming patient as a whole person; (2) Co-creating tailored personal health plan; (3) Inter-professional teamwork collaboration person his/her relatives; (4) Building foundation. Approaching an interpersonal inter-professional consultation focus on preventive promoting actions is prerequisite co-create optimal practice their relatives unique context.
